You may work with experienced rehabilitation specialists, such as:
Coordinated Care
Work with our nurse case manager and social worker to create a plan to help you safely return home or find the next level of care. Ask about home healthcare services. You’ll learn how to care for yourself when you're at home and your loved ones will learn how to support you during recovery.
