Community Health Needs Assessments
Every three years, we assess the health status of our communities. Our Community Health Needs Assessment reports contain data, maps, and community input on a variety of topics including preventative care, chronic disease and leading causes of death.

Community Health Implementation Strategy
In response to Community Health Needs Assessment findings, PIH Health develops a plan outlining how we’ll address the identified health needs of our communities.
